One of the amazing affiliates Supportive Concepts for Families (SCFF), is a leading provider of services to individuals with Behavioral Health/Intellectual and/or Developmental Disabilities in Pennsylvania.

Supportive Concepts is currently seeking qualifies employees in the Harrisburg and Susquehanna Township Area to support individuals with Developmental Disabilities both in the residential and community setting.

Responsibilities:
Proper and accurate completion and timely submission of required paperwork as required by State and Federal Regulations, SCFFI, and HIPAA including but not limited to:
On-call progress notes, biweekly case notes, consumer assessments, purchase orders, census logs, fiscal forms, maintenance forms, residential change notification forms, quarterlies, IPP, medical forms, goal documentation, community life activities, lifetime medical history, monthly progress notes, weekly progress note review.
Completion of all necessary DPW/HCFA annual requirements, Food Stamps(waiver)Social Services (SSI, SSA), Re-certification of needs (PAI962, Physical, OTC.
Attendance to scheduled meetings including:
Department meetings, provider meetings, agency and committee meetings, staff and IC meetings, consumer and family team meetings, quarterly family meetings, Community Specialist supervision, and consumer quarterly meetings.
Monitor overtime and hours worked.
Conduct orientation for new hires and employees transferring into new programs.
Ensure In home orientation is completed with direct care employees and forwarded to the Training Department.
Ensure all consumers are adequately staffed at all times.
Schedule respite as needed -maintain van schedules and update schedules as changes occur.
Transport individuals to appointments and other activities.
Maintain documentation of daily activities using an online system.
Effectively communicate with other team members and/or family.
Assist with activities of daily living such as cooking, cleaning, meal preparation and laundry.
Job Requirements:
A Bachelor's Degree with two (2) years of experience in the IDD field or an Associate's Degree with four (4) years of IDD experience is required.
Knowledge of 6400/6500 regulations is a plus, but not required.
Must pass drug screening, physical, and TB tests.
